For the person receiving care

You may be eligible if you:

  • Are 60 years of age or older

    Some home and community-based programs are designed for older Ohio residents.

  • Have Medicaid coverage

    Medicaid eligibility may be required for certain waiver-funded care options.

  • Have or may qualify for PASSPORT or MyCare Ohio

    Treasure Healthcare can help you understand the next step when waiver enrollment is not yet complete.

  • Live in Painesville, Cleveland, or a nearby community

    Submit the form even when you are unsure whether your exact city is within our current service area.

  • Can direct your care and services

    When you cannot direct your own care, you may appoint an authorized representative who can assist with decisions.

For the chosen caregiver

Your caregiver may be hired if they are:

  • At least 18 years old

    The caregiver must meet applicable employment, program, and onboarding requirements.

  • A qualifying relative

    A daughter, son, sibling, extended family member, or another eligible relative may be considered.

  • A trusted friend or existing caregiver

    The patient may identify someone they already know and trust, subject to program approval.

Get assessed

For the PASSPORT waiver, the Ohio Department of Aging or its local program administrator will assess the individual to determine eligibility. For the Next Generation MyCare Ohio waiver, the individual begins by selecting or contacting an available managed care organization.

Choose a caregiver

Choose someone you know and trust, such as a daughter, son, relative, friend, or existing caregiver. Treasure Healthcare will help explain enrollment, onboarding, and applicable program requirements.

You receive care — they receive payment

You receive approved care from someone familiar and trusted. After authorization and successful caregiver onboarding, the caregiver may be paid for properly documented and approved services.

Can a family member be paid to provide care?

In some programs, an eligible relative or trusted person may be approved and hired to provide authorized care. The specific relationship and requirements depend on the patient’s waiver, coverage, care plan, and program rules.

Can the patient choose a friend instead of a relative?

A trusted friend or existing caregiver may be considered when permitted by the applicable program. The caregiver must complete required employment, screening, documentation, and training steps.

Does the patient need Medicaid?

Medicaid is commonly required for Medicaid waiver programs. Treasure Healthcare also works with several coverage and payment arrangements, so people who are unsure should still submit an eligibility request.

What if the patient is not enrolled in PASSPORT or MyCare Ohio?

The patient may still submit the form. Treasure Healthcare can review the information and explain which organization or program the patient may need to contact for an assessment.

Does the caregiver need previous healthcare experience?

Previous experience may be helpful but is not always required. The caregiver must still meet applicable hiring, background-check, training, competency, and program requirements before providing paid services.

How quickly can the caregiver begin working?

Timing varies. The patient may need an assessment and authorization, while the caregiver must complete onboarding requirements. Paid services cannot begin until all required approvals are in place.

How and when will the caregiver be paid?

Once hired and authorized, the caregiver is paid through Treasure Healthcare’s regular payroll process for approved, properly documented services. Exact timing and compensation depend on the assignment and applicable program requirements.

Does submitting the form guarantee eligibility?

No. The form allows Treasure Healthcare to review the situation and contact the person who submitted it. Final eligibility, coverage, authorization, employment, and payment depend on applicable assessments and program requirements.
